Our team includes 2 experienced Nurses, a Physiotherapist, an Occupational Therapist, Complimentary therapists, and Counsellors. At Longfield we aim to educate, empower and enable patients, carers and the bereaved to live life to the full. We offer support in understanding and expressing feelings, provide information and advice, improving people’s well-being, management of symptoms and providing a safe space to build friendships.
Referral Criteria:

Longfield supports adults over the age of 18 with a progressive life limiting condition such as cancer, advanced lung disease, neurological diseases (including Parkinson’s, Motor Neurone Disease, Multiple Sclerosis, early to mid-stage dementia) and heart disease, who live in Gloucestershire or have a GP registered in Gloucestershire.
- Our Services are also open to carers and the bereaved, enabling people to live well and die comfortably, and to feel supported throughout and in bereavement.
When to refer:
- From point of diagnosis, onwards and into bereavement
Who can refer:
- We welcome referrals from all health and social care professionals
- Charitable sector
- We also accept self-referrals from patients, carers and the bereaved
